Donation request

CHS SunPrairie does not sponsor local recreation teams (basketball, baseball, softball, etc). Donation requests for high school athletics will be reviewed on a case-by-case basis and will be determined based on budget and location of the school.

To apply:

• Download the donation request form and complete. Please provide all requested information. Only complete applications will be considered.
• Download a W-9 form and complete. A completed W-9 must be submitted for any donation or sponsorship request to be considered.
• Email your completed forms to [email protected].

CHS SunPrairie will be in contact with you regarding your donation request within 1-2 weeks.
